PREPARING FOR YOUR PHYSICAL

A DOT physical at South Main Chiropractic takes about 30 minutes. Bring essential documents, including photo ID, medical records, medication list, eye glasses, hearing aids, and relevant medical test results and special forms such as MCSA-5870, MCSA-5871 Or exemption forms.
